All prospective members of the Association of Ukrainians in Great Britain Limited are required to complete this registration form.
You must have the right to reside in the UK to apply to be a member. Your details will be kept in a secure database with access restricted to authorized personnel only.
Whilst most questions on this form may be completed in English, we ask you to pay particular attention to ensure that you accurately respond to the questions on Names(s), Surname and Maiden Name (if applicable) in both Ukraninian and English. If you need assistance with the Ukrainian language, please do not hesitate to ask your local AUGB Branch chair or secretary.
The reason that we are asking for both is to ensure that the correct spelling is used in both cases. A Ukrainian surname written in English might not always be correctly interpreted into Ukrainian and vice-versa. Similarly, we ask you to ensure that your first name(s) are registered as they are recorded on your birth certificate or passport and NOT as you might be known locally by your friends (eg Anna in your passport but known locally as Hania). Please also use the native language when providing your place of birth (eg English if born in the UK, Ukrainian if born in Ukraine, etc).

Declaration: As a member of the Association of Ukrainians in Great Britain Limited (AUGB) I agree to abide by its Memorandum and Articles of Association and to comply with these and any other supplementary rules or decisions approved of by any General Meeting of the AUGB, its board of Directors or local Branch Committee.
